New Patient Information
REMINDERS FOR NEW ALLERGY PATIENTS

Payment is required at the time of the visit.

No antihistamines (allergy or cold medicines) should be taken for 3-5 days prior to visit. Click here for Antihistamine list.

Please print out the “New Allergy and Asthma Patient Questionnaire”, fill it out, and bring it with you to your initial appointment.

Provide your referring and/or primary physician’s name(s), address(es), and telephone number(s) so that we may effectively communicate.

Bring old records, previous laboratory results, and x-ray films, if appropriate.

Bring all medications, including spacer devices, currently in use.

For patients under 18 years of age, a parent or legal guardian must accompany the child to provide an accurate history, consent, and support for the child. If you designate another adult to give consent for medical treatment, you must provide them with written permission to make medical decisions on your behalf. The document must also include the name of the patient, the designated adult, and the date of service. The designated adult must provide picture identification.

If English is not your first language, please let us know when you make your appointment, and arrange to bring a translator if possible. A complete and accurate history is essential to providing the best care possible for you or your child.

Prepare for a thorough evaluation! The first visit will include time for a detailed history and physical examination, selected testing procedures (e.g. allergy skin tests or spirometry (breathing tests) in our office), discussion of the diagnosis, an outline of the treatment strategy, and some educational materials. For children, feel free to bring favorite toys and books.

We may need to plan a second visit to complete the evaluation and educational plan for care.

We make every effort to run on schedule, but at times, circumstances out of our control make this impossible. We appreciate your patience during those times. Please rest assured that you will receive the same level of attention and care during your own visit.
